Membership Agreement

1. Plans and benefits

Family add-ons (Essential and Executive only): spouse or partner, and up to three children, at the rates shown during enrollment. Each family member receives the same benefits as the primary member's plan.

Periodontal maintenance add-on (Essential and Executive, $15/month or $150/year): for members Dr. Noland has diagnosed with periodontal disease, the plan's included cleanings are replaced by four periodontal maintenance visits per year, scheduled approximately every three months. The add-on covers maintenance visits only. Scaling and root planing ("deep cleaning"), periodontal surgery, antibiotics, and other periodontal treatment are not included in any plan; they are billed as treatment and receive the plan's member discount.

Diagnosis after enrollment. Regular cleanings (prophylaxis) are not an appropriate service for a patient with active periodontal disease. If a member is diagnosed with periodontal disease during their membership, the office will add the periodontal maintenance add-on and the member's price will increase by $15 per month ($150 per year) starting with the next billing date; the member is told at the time of diagnosis. Members acknowledge this at enrollment. A member who does not wish to add periodontal maintenance may cancel under Section 4.

Included cleanings and exams do not roll over from one membership year to the next, and benefits are not transferable to another person.

2. Forever Smile waiting period and eligibility

Noland Forever Smile is an All-on-X maintenance plan and is available only to patients whose full-arch (All-on-X) surgery was personally performed by Dr. Conner Noland. It is not available for full-arch work done by any other provider, even if the prosthesis is now maintained at our office. Eligibility and arch count are confirmed against our surgical records at enrollment; an enrollment that does not meet this requirement will be cancelled and any payment returned.

Minimum term. Forever Smile has a 12-month minimum term from the date of enrollment. Monthly members who cancel before 12 months remain responsible for the remaining months of the initial term.

Remake coverage. Cleanings, maintenance visits, and screw changes begin immediately. Prosthetic remake coverage begins 90 days after enrollment for members who enroll within 12 months of their final prosthesis delivery; for members who enroll later, remake coverage begins after 12 months of continuous membership. Remake coverage is limited to one remake per arch in any five-year period and requires continuous membership at the time of the remake. Remakes are covered for structural failure or a clinically significant change in fit as determined by Dr. Noland; they do not cover loss, elective changes in shade or shape, or damage from misuse or neglect.

3. Billing and renewal

• Monthly plans are billed automatically each month to the card on file. Annual plans are billed once per year.
• Your membership renews automatically until you cancel. We will notify you at least 30 days before any price change.
• If a payment fails, we will retry it. If it cannot be collected, benefits are paused until the balance is brought current. Treatment discounts do not apply while an account is past due.
• Payments are processed securely by Stripe. Our office never stores your full card number.
• Invited free trials. The office may, by personal invitation only, offer a free first month. A payment method is collected at enrollment; the first charge occurs when the trial ends, and the membership then continues at the plan's regular price unless cancelled before the trial end date. Trial members receive plan benefits during the trial; one trial per patient. Stripe sends a reminder email before the trial ends.

4. Cancellation and refunds

• You may cancel at any time by calling (501) 362-0550 or emailing our office. There is no cancellation fee. Cancellation stops future charges; benefits continue through the end of the period you have already paid for.
• Monthly plans are non-refundable. No refunds are issued for the current or any previous month.
• Annual plans are non-refundable, with one exception: if you cancel within 30 days of payment and have not used any included service, we will refund the annual payment in full. Once any included service has been used, or after 30 days, annual payments are not refundable and benefits continue through the end of the paid year.
• Membership fees are not refunded or credited if you move, change providers, or do not use your included services.
• Forever Smile: the 12-month minimum term in Section 2 applies; cancellation before 12 months does not relieve the remaining months of the initial term.

5. Treatment discounts

• Amount and cap. Essential Care members receive 10% off our standard fees for treatment, up to $2,000 in total savings per membership year. Executive Care members receive 15% off, up to $3,000 in total savings per membership year. Once the cap is reached, further treatment in that membership year is at standard fees.
• All-on-X and full-arch treatment is excluded. The membership discount does not apply to All-on-X / full-arch implant treatment, including surgery, implants, temporary and final prostheses, and related laboratory work, under any plan. (Noland Forever Smile members receive that plan's 15% off revision surgery as described in Section 1.)
• Major treatment and minimum term. Applying the member discount to any treatment plan of $2,500 or more (before discount) commits the member to 12 months of membership from the date of that treatment. A member who cancels before that date remains responsible for the remaining months of the term, which the office may charge to the card on file.
• Discount recapture. If a membership is cancelled by the member, or lapses for non-payment, within 12 months of receiving discounted treatment, the discount received on that treatment becomes due and may be charged to the card on file.
• Discounts apply to our standard fees and cannot be combined with dental insurance, other discount programs, or promotional offers. If you have dental insurance, you may use either your insurance or your membership for a given procedure, but not both. Discounts do not apply to outside laboratory fees, specialist referrals, or products billed by third parties.
• By enrolling, the member authorizes the office to charge the card on file for amounts due under this section and under the Forever Smile minimum term.

6. Appointments

Included visits must be scheduled and completed at our office. Included cleanings and exams are intended as routine care and are scheduled at least six months apart (about every three months for members with the periodontal maintenance add-on) unless Dr. Noland determines a shorter interval is clinically necessary. Whitening trays are made once per membership; replacement trays and additional gel are available at member pricing. Whitening is not recommended for every patient; Dr. Noland will advise if it is not appropriate for you. Missed appointments without 24 hours' notice may count as a used visit. Benefits begin on the date of enrollment and the membership year runs 12 months from that date.

7. Changes to the program

We may update plan benefits or terms with 30 days' written notice. If we discontinue the program, we will refund any prepaid, unused portion of an annual membership on a prorated basis.

8. Privacy

Information you provide during enrollment is used only to administer your membership and your dental care, and is protected under our Notice of Privacy Practices.
